This page explains how website form submissions and scheduling information may be handled. It is not a substitute for individualized clinical or legal guidance.
Getting to Know You Form submissions
The Getting to Know You Form collects the parent or guardian’s contact details, the child’s age in months or years, a reason for reaching out, and the general message you choose to send. Do not submit a child’s name, date of birth, diagnosis, medical history, records, insurance information, or Step Up account details.
Wix storage and hosting
When enabled, form submissions may be stored through Wix hosting and a private Wix data collection so the practice can review and respond. Hosting systems may also process limited technical information, such as browser type, device information, IP address, and pages requested, for normal operation and security.
Calendly scheduling
The Book a Call page uses Calendly, a third-party scheduling service. If you use it, Calendly receives the information you submit and handles it under its own terms and privacy practices. Please enter only the information needed to schedule a general call.
Children’s information
This public website is intended for parents and caregivers. The general form is not a clinical intake and should not be used for urgent or sensitive medical information about a child. A future full intake may use a separate secure process.
Security limitations
Reasonable safeguards can reduce risk, but no website or internet transmission can be guaranteed completely secure. Use care when deciding what to send electronically, and contact local emergency services for urgent or emergency concerns.

Use the general contact form for a privacy question, without including sensitive information. We may update this policy as the website and services change; the current version will be posted on this page.
